0.3 mm is about as fine as a working tip gets, and at that size the limiting factor is no longer the tissue — it is whether the two tips actually meet. Arms slim enough to end in a 0.3 mm point are also slim enough to deflect sideways under finger pressure, and once the tips pass each other the instrument grips nothing. The stabilising pin prevents exactly that, constraining the arms to close without lateral movement.
The tying platform behind the tips is the other half of the design. Suture fine enough to need a 0.3 mm tip cannot be tied against the points themselves — there is no surface there. A flat milled area a few millimetres back gives the thread something to be drawn across and tensioned on, so the same instrument positions the suture and then ties it without the surgeon looking away from the microscope.
