A splinter presents a particular problem: almost all of it is under the skin and only a fraction is available to grasp. The spade tip is the answer — it broadens just behind the point, so the forceps can be worked down alongside the fragment and closed on whatever length is exposed, gripping across a face rather than pinching at a point. A plain pointed forceps slips off the same fragment or crushes it, and a crushed splinter has to be dug out instead of drawn out.
The serrated handle carries the fine control that follows. Removal is a slow steady pull along the axis the fragment entered on; pull at the wrong angle and it snaps, leaving the remainder deeper than before. Serrations give the fingers purchase for that controlled traction. At 10.8 cm the forceps are short and held like a pen, which is the grip precision at this scale needs.
