Carrying both scales on one ruler matters more in surgery than it looks. Instrument specifications, implant sizes and published techniques are quoted in whichever unit their source used, and converting in theatre is where errors enter. A ruler showing both lets the surgeon read the measurement in the unit the plan was written in, without arithmetic.
Rigidity is the other requirement. A ruler laid against tissue is pressed down to sit flat, and anything that bends under that pressure reads short — the scale follows the curve while the distance being measured does not. Stainless steel holds its shape under that handling and survives the repeated sterilisation an instrument used in every case goes through.
