Pin-and-aperture is a different holding mechanism from teeth or serrations. Each pin enters a matching hole in the opposite jaw, so whatever is caught between them is trapped in three places along a path it cannot slide out of. A toothed jaw resists a pull across the teeth but lets material creep along them; a pin seated in a hole leaves no direction to creep in.
The trade is that three pins engage a small total area, so this is a gripping instrument rather than an atraumatic one — chosen when something smooth and springy must not move, not when broad tissue needs supporting. The narrow profile suits the same purpose: it reaches into a confined field without occupying the view, and the ridged grip keeps the hand steady while the pins are seated.
