The fenestration does two jobs at once. It leaves the centre of the graft visible through the jaw, so the surgeon can see the edge being aligned rather than inferring its position from a solid blade covering it. And it removes the middle of the contact area, concentrating grip on the oval rim — the graft is held around its perimeter, which is secure enough to position it while sparing the central cartilage from the compression that leaves visible marks under thin skin.
The bayonet shank offsets the working tips laterally from the handle. In a nasal field that offset moves the surgeon’s hand and fingers out of the line of sight to the tips, and out of the way of an endoscope or a light source approaching the same narrow opening. The 6 mm jaw width is sized to the grafts used in tip and dorsal work.
