Left and right versions exist because the mandible is a mirrored structure and the approach is one-sided. A retractor shaped to hug the medial surface of the right ramus meets the left one at the wrong angle — it contacts on a corner instead of bedding along the bone, so it slips under load and holds nothing. Matching the side is what lets the blade seat flat and stay there.
At 19.5 cm this is shorter than the 29 cm midface retractors, matched to an intraoral approach where the bone sits only a few centimetres behind the incision. Length beyond what the approach needs is not neutral on an intraoral instrument: it projects past the lips and competes with the drill, the elevator and the assistant’s hands working through the same small opening.
