The medial ramus and the orbital rim sit at opposite ends of the midface, but they present the surgeon with the same difficulty: both are approached along a narrow corridor with bone on one side and structures that must not be disturbed on the other. A retractor shaped for that geometry — long, slim, and angled to sit against bone — works in either location, which is why the pattern carries both names.

The orbital rim is reached from a remote incision to keep scars off the face, and the medial ramus lies deep behind the cheek; in both the handle has to stay well outside while the blade works at depth. Multiple sizes exist because the corridor width varies with the approach and the patient, and a blade wider than the corridor has to be forced — which loses the seating against bone that makes the retractor hold.
