The curved handle moves the hand out of the working line. In facelift and breast surgery the dissection runs beneath a raised flap, and the surgeon looks along that plane — a straight handle puts the assistant’s hand directly in the sightline for the whole procedure. Curving it carries the grip off to the side while the blade stays flat against the flap.
What a flap retractor is really managing is the appearance of the result. The tissue it holds is the skin the patient will see, and it is held under tension for a long time; concentrated pressure or a defined edge leaves bruising that outlasts the operation. The blade spreads its load and the polished surface releases cleanly, so the same flap can be lifted and repositioned repeatedly without accumulating marks.
