Inspection is a real step in breast surgery, not an afterthought. Once a pocket is made the surgeon needs to know whether the dissection sits in the intended plane, whether it is even across its whole area, and where it still needs releasing — all judged by running an instrument through it and reading what comes back through the handle. A round, flat blade is the right shape for that: it presents no edge or corner that could snag or cut while being swept through a plane it is only supposed to be assessing.
The dimensions follow from the submammary route. Entering beneath the breast puts the incision close to the pocket, so 29.8 cm is ample reach and a longer instrument would only be unwieldy. The 20 mm blade is broad enough to feel a plane properly but narrow enough to accommodate a range of patient sizes, including smaller pockets where the 38 mm transaxillary blade simply would not fit.
