A skin hook is held under continuous tension, and the one thing that must not happen is the handle turning in the hand. A smooth handle does exactly that once it is wet with saline and blood — and when the handle rotates, the hook rotates with it and either releases the tissue or drags through it. Knurling gives the fingers a mechanical grip that does not depend on friction against a polished surface.
That matters most in the work this pattern is named for. Kleinert and Kutz are associated with hand and microsurgery, where the structures being retracted are small and the tension holding them is fine — a slip of a millimetre is a real error rather than a nuisance. At 16 cm the hook is long enough to keep the hand outside a small field while remaining short enough to be controlled from the fingers.
