The choice between the single and double Joseph hook is about the length of edge being held, not about strength. A single prong holds one point precisely, which is right for lifting a corner or a short segment. But apply that same prong along a longer stretch of skin edge and the tissue either side of it falls away, so the margin curls and the surgeon is working against a moving line. Two prongs fix the edge at two points and hold the segment between them flat.
Sharing the pull also matters once the tension goes up. A single fine prong under a firm pull through a long dissection will eventually tear through facial skin; two prongs halve the force at each point while the total traction stays the same. At 16 cm the hook is slightly longer than the single-prong pattern, giving reach across a wider plastic or reconstructive field while the tips stay under finger control.
