The trachea is a mobile structure. It rides up and down with respiration and swallowing, and it rolls away from an instrument pressed against it — which makes an accurate incision into it difficult without something holding it still first. A tenaculum engages the cartilage at a single point and fixes the airway in position, so the surgeon cuts into a stationary target.

A single hook rather than a grasping jaw is what suits tracheal cartilage. Jaws applied to a tracheal ring crush the cartilage between them, and crushed rings do not hold their shape afterwards — which matters for a structure whose only function is to stay open. A fine point engages between or through a ring without compressing it. At 14.5 cm the instrument is short, matched to the shallow depth of a tracheal approach at the front of the neck.
