This instrument does not grip bone — it grips the wires. K-wires placed either side of a fracture or an osteotomy become fixed handles on the bone fragments, and the distractor engages those wires through its pin holes. Opening it then pushes the fragments apart in a controlled, measurable way, with the force applied through the wires rather than through anything clamped onto the bone surface.
That distinction matters where the bone is small or already compromised. A clamp or a lever bears directly on cortex that may be thin, fractured or osteoporotic; wires transmit the same force through fixation points chosen deliberately by the surgeon. Two pin sizes, 1.6 mm and 2.4 mm, cover the common wire gauges so the instrument matches what is already in the patient rather than requiring a particular wire to be planned in advance.
