Open rhinoplasty gives the surgeon a working space roughly a centimetre across, and an assistant holding the columellar flap occupies a good part of it. The weight removes that: it rests off to the side of the field and its pull holds the blade against the flap, so the exposure maintains itself and both of the surgeon’s hands stay free.
Self-retaining also means constant. Tension from a hand varies as the assistant shifts and tires, and the flap moves with it — awkward when tip projection and dorsal height are being judged by millimetres against that flap as a reference. A fixed weight applies the same tension from start to finish. The weight is stainless steel, so the whole instrument reprocesses as one piece with nothing to detach or handle separately. The blade is narrow at 7 mm because the structure it holds is narrow; anything wider would press on the alar cartilages either side of the columella.
