The length of this instrument is dictated by the approach it was designed for. In the Gillies technique the incision is made in the temporal hairline, well away from the face, and the elevator is passed downward beneath the temporalis fascia until its tip lies under the depressed arch. The fracture is then lifted from below through an incision that leaves no visible scar. 23 cm is what it takes to span that distance and still leave the handle outside the wound.
The blade has to be slim to make that passage — it travels through a tissue plane rather than an open cavity, and a thick blade would have to force its way, stripping tissue it should be sliding past. At the same time the 9.0 mm width and 35.0 mm working length give enough surface to spread the lifting force across the underside of the bone. A narrow point would concentrate that force and punch through the arch rather than raise it.
