A double hook sits between a single hook and a rake. One point concentrates all the traction where it enters; four points need a wound edge long enough to place them all. Two points spread the load across a short span, which is the common case in facial and reconstructive work — a segment of skin edge a centimetre or two long that needs steady tension and would tear on a single hook.
3 mm sets how much tissue each hook takes. Narrower and the hooks would tend to pull through under tension; wider and they would need more force to seat, which on thin tissue means pressing rather than engaging. At 15 cm the instrument works at moderate depth, longer than the short skin hooks used at the surface but still controlled from the fingers rather than the wrist.
