Putting two diameters on one instrument changes how the size is chosen. With separate markers the surgeon lays one on, sets it down, picks up the next and compares from memory. A double-ended marker is simply reversed — both sizes are in the same hand, and the comparison is immediate rather than remembered. When the decision is between two adjacent diameters, that is the whole difficulty resolved.
The pairings are chosen as adjacent steps rather than as extremes: 34/36 and 36/38 at the smaller end, 42/45 and 45/50 at the larger. Each instrument therefore covers one decision point in the range, and a surgeon can carry the two or three pairings that span their usual work instead of a full set of single markers. It also halves what sits on the tray for the same coverage.
