Diced cartilage is used for dorsal refinement because it conforms to the pocket it is placed in and leaves no palpable edge — but only if it can be delivered without disturbing the pocket. This trocar loads the diced graft and injects it through a small entry point, so the recipient site stays closed around the material. An open placement, by contrast, requires a dissection wide enough to admit an instrument, and a pocket wider than the graft lets the cartilage migrate laterally and settle off the midline.
Delivering through a trocar also makes the augmentation incremental. Material can be introduced in stages with the dorsum assessed by palpation between passes, which matters because diced cartilage settles and the endpoint on the table is not the endpoint at six months. Adding a small amount and reassessing is far easier than removing an overcorrection from a closed pocket.
