A tenaculum solves a problem forceps cannot. Grasping tissue between two jaws applies pressure across the whole contact area, and soft, vascular tissue crushes under enough of it to hold securely. A single sharp point penetrates instead — it engages at one small site and holds against real traction, while the tissue either side of that point is untouched.
That trade is worth making when what is being held has to be pulled rather than merely steadied. In gynaecological and obstetric work the tenaculum takes traction on a structure so it can be drawn down into reach, and the pull involved would tear tissue out of any atraumatic grip. At 22 cm the instrument reaches well into a deep field with the hand outside, and a fine point rather than a blunt one is what makes the initial engagement possible on a smooth, moist surface.
