This works in the opposite direction to a zygoma elevator. An elevator is passed beneath the arch and lifts it from below; a hook is placed over or around the bone and pulls it outward and forward. Which is appropriate depends on how the fracture is displaced — some arches need lifting off the underlying structures, others need drawing out laterally, and no single direction of force reduces every pattern.
The heavy build is not a marketing word here. Reducing an impacted zygoma takes real force applied through a narrow point of contact, and a slender hook under that load either springs — losing the surgeon’s feel for whether the bone has moved — or deforms permanently. A 9.0 mm hook width spreads the pull across enough bone to avoid cutting into it, while the sharp point is what allows the hook to engage the arch at all rather than skidding across its surface.
