A Gelpi is self-retaining — it is placed, opened and locked, and then holds by itself. That is the whole reason it exists: in a deep narrow field there is often no room for an assistant’s hand at all, and even where there is, a held retractor drifts as the assistant tires. A ratcheted retractor applies exactly the same tension from the first minute of the operation to the last.
Angling the prong at 90° lets it engage tissue at the bottom of a deep wound while the arms of the retractor stay above it. A straight prong on a deep retractor has to be pushed in along the same axis the surgeon is working down, so the body of the instrument sits in the corridor it just opened. Turning the prong through a right angle moves the whole mechanism to the side. The prong is sharp because a self-retaining retractor must not slip — there is no hand on it to notice and correct if it does.
