The DeBakey jaw grips without teeth — fine interlocking longitudinal ridges hold vessel wall between two textured faces rather than piercing it. That is the standard for vascular work, because a puncture in an artery is a repair and a crushed segment fails days later, neither visible at the moment the forceps closes.
What this version adds is reach into tight spaces. Cardiovascular exposures are deep and bounded by structures that cannot be retracted further, so a broad instrument simply cannot be angled onto the vessel. Narrowing the whole profile lets the same atraumatic jaw be brought to a vessel lying behind other structures — without the width that would force the surgeon to enlarge the exposure to admit the instrument.
