The two ends do opposite things, and that is the design. The 4.0 mm sharp blade cuts: it starts incisions and works through the tougher, more fibrous attachments where tissue will not separate by pressure alone. The 3.0 mm round spatula does the reverse — blunt and smoothly edged, it slides between layers and lifts them apart without dividing anything.
Alternating between the two is how septal and reconstructive dissection actually proceeds. Most of the plane opens bluntly with the spatula; every so often the dissection meets a firm attachment that will not yield, and forcing the spatula there tears surrounding tissue instead of releasing the attachment. Reversing to the sharp end divides that one point, and the blunt end resumes. Having both on a single 22.9 cm shaft means that switch costs a turn of the wrist rather than an instrument exchange.
