A coronal flap is wide, heavy and raised across the top of the scalp, and holding it with one or two prongs concentrates the whole load at those points — which tears through scalp under sustained traction. Seven prongs across 70 mm divide that load into seven small shares, so each engages the tissue lightly while the flap as a whole is held firmly. The prongs are sharp because scalp is dense and elastic; a blunt prong under this much tension simply slides out.
The offset design moves the handle out of the plane of the blade, so the hand holding a wide scalp flap back sits above and behind the working area rather than across it. In brow lift surgery the dissection runs forward beneath the flap toward the orbital rims, and the surgeon has to look along that line — an in-line handle on a 70 mm retractor would sit directly in that sightline for the whole procedure.
