6 mm and 11 mm is close to a doubling, and that spread is the point of carrying both on one shaft. The wider end holds a useful amount of tissue and is the one reached for by default; the 6 mm end exists for the parts of the same procedure where there is simply not room for it — between structures, at the depth of a narrowing exposure, or where a wider blade could only be seated by stretching the opening.
Forcing an oversized blade is not a neutral compromise. It stretches the wound edge, which bruises and swells, and it seats poorly so it slips under traction just when the exposure is needed most. Reversing to the narrow end costs a turn of the wrist. At 18 cm the retractor reaches moderately deep fields while the handle stays outside, and the balanced double-ended form means neither end is awkward to hold.
