This instrument is named for two jobs because it does both with one blade. The blunt 6 mm edge is slid along bone to separate periosteum from the surface beneath it, and once that flap is raised the same blade is turned and used to hold it clear while the surgeon works. In a deep oral or maxillofacial field that combination removes an instrument exchange at the exact moment the flap is most likely to fall back into the field.
Blunt edges are essential to the elevating half of that job. Periosteum separates cleanly only when the instrument stays in the natural plane against bone; a sharp edge cuts through the layer instead of lifting it, and the flap tears rather than peeling. At 24 cm the instrument reaches well into the mandible or a deep orthopaedic exposure while the handle stays outside the wound, and both ends give the surgeon a choice of approach angle without repositioning the hand.
