The channel is what distinguishes this from a plain blade retractor. A flat blade held against the lower border of the mandible is kept there only by the surgeon’s pressure, and the moment traction increases it rides off the bone into soft tissue — losing the exposure at the point in the operation when it is most needed. A channel formed along the blade fits over the border itself, so the retractor is captured by the bone and holds its position under load.
Two lengths exist because the mandible is not one working distance. 18 cm suits the body and symphysis, where the bone sits close to the incision; 22.5 cm reaches the angle and ramus, which lie deep behind soft tissue and muscle. Choosing the shorter instrument where it fits keeps the handle out of the field — excess length on a retractor that is held for the whole procedure simply gets in the way of every other instrument.
