Packing and dressing material inside a nose or ear behaves nothing like tissue. It is bulky, light and compressible, so the jaws need surface texture rather than teeth — serrations hold gauze and ribbon packing by friction across a face, where a tooth would simply pierce through it and let it slide off. Keeping the serrations delicate matters because the same jaws touch mucosa on the way in and out.
The bent bayonet handle is what makes that possible in a cavity. Placing packing means looking directly into a narrow opening and feeding material to the far end, and an in-line instrument fills the very opening the surgeon is sighting through. At 14 cm the instrument is short for a bayonet, which suits ENT and superficial work rather than the deep fields the longer bayonet patterns are built for.
