The condylar neck sits at the top of the mandibular ramus, deep behind a mass of soft tissue, and reaching it means holding that tissue aside for the whole operation rather than momentarily. A retractor for this region has to do two things at once: keep the corridor open and stay put while the surgeon works with both hands. A contoured blade does the second part — shaped to the bone it rests against, it seats and holds instead of sliding off under the tension of the tissue it is holding back.
