A 75 mm blade on a 75 mm instrument means this is held in the palm or between the fingers at the blade itself, with nothing projecting. That is exactly right for intraoral work: the mouth is a small space already occupied by the surgeon’s other instruments, and a retractor with a handle sticking out of it competes for the same room and fouls the drill or the elevator coming in from the other side.
What it holds back varies constantly — cheek, lip, tongue, a mucoperiosteal flap — and all four are soft, mobile and pressed against for the length of the procedure. Smooth contours across the full 15 mm width are what make that tolerable: the load spreads along the blade rather than concentrating at an edge, so buccal mucosa held for an hour is not marked by the instrument holding it.
