Light alone does not keep a nasal field visible. Blood collects in a closed cavity faster than it drains, and an illuminated pocket full of blood is no more workable than a dark one — so this version routes suction through the retractor itself. The cannula clears the field continuously at the point the blade is holding open, rather than requiring a separate sucker introduced through the same small opening.
The 5 mm hole in the blade is where that suction acts, positioned so it draws from the working area rather than from wherever the tubing happens to point. At 20 cm the instrument is longer than the plain fibre optic version, reaching further back along the nasal cavity — which is where fluid pools and where a shorter retractor stops short.
