The gap between 40 mm and 42 mm is the smallest in this range, and that is the point of having both. Areola size is judged against breast volume and chest proportions, and the difference between one size and the next needs to be fine enough that the surgeon is not forced into a noticeable jump. Two millimetres of diameter is close to the limit of what is perceptible, which is exactly the resolution a template set should offer near the middle of its range.
Because the choice is often made on the table rather than beforehand, the marker has to work against tissue that is already prepared. Blunt edges allow it to be pressed and repositioned on skin without leaving an impression of its own, so a size can be tried, assessed and changed before anything is committed to.
