An areola marker is a template, and the size chosen sets what the patient sees afterwards. At 36 mm this is the smallest in the range — the choice where a smaller final areola suits the proportions being created, or where the surgeon expects the areola to widen slightly under tension once the breast is closed and wants the marked outline to allow for it.
Marking with a fixed template rather than freehand is what makes the two sides match. Both areolae are outlined against the same rigid circle, so any difference between them comes from where the marker was placed rather than from how steadily each outline was drawn. The edges are blunt because the instrument is pressed against skin that is about to be incised, and a defined edge leaves its own mark alongside the intended one.
