215 g is a deliberately small mass. In cervical work the consequence of an osteotome advancing further than intended is measured against a spinal cord millimetres away, so the instrument is chosen to make each strike a small increment rather than an efficient one. A heavier mallet would finish the same cut in fewer blows, but each of those blows would carry more energy than the margin for error allows.
The 19 cm handle shortens the arc of the swing to match. A short handle limits how much speed can be generated even with a full wrist movement, which caps the energy of an accidental hard strike — the mallet is built so that the worst blow it can easily deliver is still a small one. Solid head construction keeps the feel of each contact crisp, so the change in resistance as bone thins is transmitted to the hand.
