Energy delivered to an osteotome is a product of head weight and speed, and the two are not interchangeable in surgery: a light head must be swung fast to do the same work, and a fast swing is harder to stop accurately. A heavy head advances the chisel on a short, slow, controlled tap, so the surgeon governs depth by how many taps are given rather than by how hard each one is.
The head is solid rather than filled or faced. A solid head returns a sharp, consistent impulse and — more usefully — a clear change in sound and feel as the osteotome passes from cortical into cancellous bone or approaches a far cortex. That feedback is a working depth gauge, and a damped or composite head blurs it. The 22.5 cm handle sets the balance point so the mass sits forward, over the striking face.
