Placing a K-wire through a cartilage graft freehand is harder than it sounds. Cartilage is firm and the wire wanders as it enters, so a wire aimed at a point often emerges somewhere else — and a graft with a wire through the wrong axis is bent or split rather than fixed. A jig holds the graft and presents the wire at a defined position and angle, so where it enters determines where it exits.
Multiple holes let one jig serve several configurations. Fixation may need one wire or two, placed along the graft or across it depending on what is being stabilised, and each hole is a predetermined option rather than a fresh judgement. In rhinoplasty, where the graft is going under thin skin, a wire placed slightly off axis is not a minor error — it shows.
