A mapping pen does what a template cannot: it draws whatever line the anatomy calls for. Templates and markers produce a fixed shape — a circle, a straight edge — which is exactly right when the shape is standard and useless when it is not. Mapping an excision margin or a flap outline means following the lesion or the anatomy, and that line exists only on the patient in front of the surgeon.
The nib width decides how much of the plan survives the cut. A broad mark is a band a millimetre or more across, and the surgeon then has to decide whether to cut its inner edge, its centre or its outer edge — on a dermatological excision margin that ambiguity is the margin. A fine nib lays down a line narrow enough that the question does not arise. At 14.5 cm the pen is held like a writing instrument, which is how the fine control is achieved.
