The two ends do opposite halves of the same step. One holds tissue back; the other controls where the blade goes. Combining them means the surgeon retracts, reverses the instrument and cuts along a guided line without setting anything down — and in a small field, every instrument not on the tray is room gained.
A closed knife guide is the safety half. The blade runs inside a channel rather than along an open edge, so it cannot wander laterally into tissue the surgeon did not intend to cut. 4.0 mm sets how much blade is exposed. The retractor end is blunt and rounded at 15 mm wide, which spreads the hold across enough skin to be stable without marking it.
