230 mm of blade is a different order of instrument from the millimetre-scale retractors used in facial and nasal work. At this width the blade holds back a whole segment of abdominal wall, and the reason for the size is that anything narrower simply lets the tissue fold around its edges and close the exposure again. Width is what keeps a large cavity genuinely open.
Single-ended rather than double follows from the same scale. A blade this large gives the instrument all the weight and leverage it can usefully carry, and a second blade at the other end would make it unwieldy without adding anything — abdominal retraction is held in one orientation for long stretches, not switched between angles. What matters instead is a broad smooth surface, since the blade bears against bowel and peritoneum under sustained pressure.
