Undermining is the step that lets a flap move. Skin advanced or rotated into a defect will not reach unless it is first freed from what holds it down, and that freeing has to happen over a much larger area than the incision itself. At 38 cm this instrument reaches well beyond its entry point, separating the plane as it advances so the release is achieved without cutting a wound the size of the area being released.
The 3 cm lift is what keeps the plane correct. Rather than sliding flat, the working end holds the skin up off the layer beneath as it travels, so the surgeon separates against a defined gap instead of pressing into tissue and guessing at the depth. Getting that depth right is the whole difficulty of undermining — too superficial and the flap is left thin and at risk, too deep and structures below the plane are damaged.
