An uninsulated metal retractor held in a field where electrosurgery is being used becomes part of the circuit. Current finds the retractor, travels along it and discharges at whatever tissue the blade happens to be pressing on — which is by definition somewhere the surgeon is not looking, since the retractor is holding tissue out of the way. The resulting burn is discovered afterwards, not during.
Insulating the instrument closes that path, so the current stays at the electrode tip where it was placed. That matters most in exactly the work this retractor is built for: microsurgical, plastic and reconstructive fields where haemostasis is done with fine diathermy millimetres from the retractor blade. Two working ends give a choice of retraction size without a second instrument entering an already crowded small field.
