Nothing on this instrument grips, and that is the design. Ocular tissue — conjunctiva, the surface of the globe, the structures behind the lids — cannot tolerate being held between jaws or engaged on a point; either would perforate or tear tissue measured in fractions of a millimetre. A spatula slides underneath and lifts or displaces by supporting from below, which applies no localised force at all.
The flatness matters as much as the smoothness. A blade with any curve or edge concentrates its contact along a line, and on a surface as delicate as the globe that line becomes a pressure mark. A flat blade beds against the tissue across its whole face. The same shape lets it act as a shield — slid between the globe and an instrument working above it, so the eye is protected by something solid rather than by care alone.
