Spreading is harder to hold than retracting. A retractor pulls tissue that gives; a spreader pushes structures apart against everything holding them together, so the force at the jaws is high and constant. Smooth jaws under that load slide off the moment the angle shifts — and a spreader releasing suddenly in a laminectomy field releases into the spinal canal. Teeth engage rather than press, so the grip does not depend on friction.
The 5 cm maximum opening sets what the instrument is for. That is a large span, enough to hold an interlaminar space widely open once the spreader is seated, and 24.2 cm of length puts the jaws at the depth of a posterior spinal exposure while the handles and ratchet stay above the wound where the surgeon can adjust them without reaching into the field.
