A fixed hook has to be re-seated every time the angle of traction changes, and each re-seating means another puncture in the skin edge. The swivel removes that: the prongs turn to follow the pull, so a single placement holds through a dissection that moves around the wound. On a long incision worked along its length, that is the difference between one entry point and half a dozen.
It also keeps the load square on both prongs. When a fixed two-prong hook is pulled off-axis, one prong takes most of the tension and can tear through while the other barely engages. A swivel lets the pair align themselves to the direction of force, so the load stays shared. At 11.5 cm the instrument is short and finger-held, which suits the superficial skin work it is designed for.
