A Hohmann works differently from every soft tissue retractor around it. Its tip is hooked over the far edge of a bone — the acetabular rim, the femoral neck — and the shaft is then levered against that bone. The bone becomes the fulcrum, so the retractor holds muscle and capsule aside with very little effort from the assistant, and holds it consistently rather than as strongly as someone happens to be pulling.
The long curve is what makes that leverage possible. It carries the tip around the far side of a joint while keeping the handle out at the skin, and 40 cm is what it takes to span a hip exposure from surface to acetabulum. The 22 mm blade is narrow for the size of the instrument, because it has to pass between bone and soft tissue rather than sit in an open field — width there would mean forcing a wider gap than the anatomy offers.
