There are two distinct jobs a hook can do, and the Guthrie is built for the less obvious one. A retracting hook holds tissue away from the field under sustained tension. A lifting hook picks tissue up and presents it — tenting a small fold of skin so it can be incised, or steadying an edge so a suture can be placed accurately. That is brief, precise work, and it needs a fine point that engages instantly rather than a heavy one built to endure long traction.
Two points rather than one keep the lifted tissue square. A single hook lets the fold rotate around its own axis, so the surgeon is cutting or suturing against a moving target; two points fix the orientation as well as the position. The German stainless construction matters most at the tip, since an instrument whose entire function depends on a fine sharp point becomes useless as soon as that point rounds off.
