The proportions here are the whole design. 38 mm of working width on a 7.5 cm instrument means there is barely a shaft: the retractor is held in the palm or between the fingers, right at the tissue. That is what a flap retractor for shallow work wants — no lever arm to amplify a shake, and nothing projecting into a field where the surgeon’s other instruments are already working.
Four prongs across that 38 mm hold a broad stretch of flap edge with the load split four ways. On a flap, evenness matters more than grip strength: a flap held at one or two points tents between them, and the raised areas are the ones that blanch and bruise. Spreading the same pull across a wide, evenly pronged edge keeps the flap flat, which is both kinder to it and gives the surgeon a truer view of how it will actually lie.
