Setting the inner prongs back changes how the load is shared. On a rake with all prongs level, a curved or uneven tissue edge contacts the outer prongs first and they take almost the whole pull — which is the situation the extra prongs were meant to avoid. Recessing the inner pair means they come into contact as the tissue tents under tension, so all four end up loaded rather than two.
That evenness is the point in facelift and mammaplasty work, where the tissue being retracted is a long skin flap and the marks left by uneven traction are visible in the result. At 18 cm the retractor holds a longer stretch of edge than the mini pattern and reaches across a wider dissection, while the prongs remain fine enough that each entry point closes without trace.
