The blade proportions here are unusual — 171 mm long against only 25 mm wide — and that ratio defines what it is for. A narrow blade passes through a small incision and along a developing pocket without needing the opening widened, while the length carries the working end, and the light with it, all the way to the far margin. A wider blade would light the same pocket but could not be introduced without a longer scar.
Fibre optics matter more here than in an open field. A breast pocket is a closed cavity and the surgeon’s own retractor blocks the theatre lamp; light delivered at the working end arrives where the blade is and moves with it, so whatever is being dissected is lit regardless of angle. In augmentation the far edge of the pocket determines how the implant sits, and it is precisely the part that is hardest to see.
