Endoscopic plastic surgery is done through openings too small to admit three separate instruments, so combining functions is not a convenience — it is what makes the approach possible. This retractor holds the optical cavity open, carries the 10 mm scope in a fixed relationship to that cavity, and suctions from the same working end. One instrument through one port does what three would otherwise require.
Fixing the scope to the retractor also stabilises the view. A hand-held endoscope drifts relative to the tissue it is imaging, so the surgeon is constantly reorienting to a moving picture; mounted on the retractor, the scope moves only when the retractor does, and the image stays registered to the cavity. Suction matters for the same reason — in a closed optical pocket even slight bleeding obscures the lens, and clearing it without withdrawing the scope keeps the procedure continuous.
